How to Direct Your Gift

When making your donation-either online or in person-you can easily choose how your gift is allocated.


Online Donation:

  • Support Both Foundations: Select the option to share your donation between the Carleton Place & District Memorial Hospital Foundation and the Almonte General Hospital Fairview Manor Foundation.
  • Support the Foundation Closest to Your Heart: Choose to direct your full donation to the hospital that means the most to you and your family.


In Person Donation:

If you’re donating in person, simply let our team know how you’d like your gift to be allocated and fill out the form required. We accept Cash, Cheque, Credit Card, or VISADebit. 


If you are donating to BOTH foundations, please make cheques payable to CPDMH Foundation with a note indicating "SHARED GIFT". Your receipt will be issued by CPDMH Foundation.

    Note that our office is closed from 12:00 pm to 1:00 pm daily.
